This guide tells you what happens if your practitioner changes or your service ends. It uses short sentences and pictures.

Why things might change

Sometimes your practitioner changes. For example, they move to a new job or a new area.

Sometimes your plan is finished, and you do not need us any more.

Sometimes your NDIS funding for behaviour support runs out or stops.

You can choose to stop working with us at any time.

Sometimes we are not the right service for you. We will talk about this with you.

If your practitioner changes

We tell you before it happens.

A Team Leader helps choose your new practitioner.

Your new practitioner reads your file and your plan. You do not have to start again.

We check that you are happy with the change. If you are not happy, please tell us.

Notice

If we need to end your service, we give you at least 2 weeks notice.

A Team Leader makes this decision. We talk with you about it first.

We write to you to explain why.

If you want to stop, please give us 2 weeks notice if you can.

A safety plan

Before your service ends, we make sure there is a safety plan.

A safety plan tells the people who support you how to keep you safe.

You can have a copy of your behaviour support plan.

Moving to a new provider

We can help you find a new behaviour support provider.

With your OK, we share your plan and information with your new provider. This is called a handover.

If you say no, we do not share it.

With your OK, we can talk with your new practitioner. This helps them get to know you.

You can come back

You are welcome to come back to us later.

You will need NDIS funding for behaviour support in your plan.

You can call the NDIS Commission on 1800 035 544.

You will not get in trouble for telling us.
